The first day of school is August 25!
Teacher provides 2 reminders and simple corrective actions.
Example: "Jonathan, I need you to remain seated"
Example: When Jonathan is standing up again, the teacher moves closer to her and quietly asks her to sit down.
Teacher contacts home to explain the behavior and intervention that will be put into place.
Example: "I've noticed that Jonathan is out of his seat during instruction. This disrupts his learning and keeps him from being as successful as possible. I am going to ask him to pass out papers periodically during class to give him the opportunity for movement with the understanding that he remains seated during the other portions of class. Thank you for your support."
If the behavior improves, the teacher calls home to let the parent know and the teacher will continue the intervention with routine monitoring.
Example: "Jonathan has been able to remain seated with the periodic opportunities to get up and pass out papers. I'm going to continue to have him do this. He seems to enjoy the opportunity for leadership in class. Thank you for your support."
If the behavior does not improve, the teacher will call home to let the parent know and inform the parent of a second intervention.
Example: "Jonathan is continuing to have difficulty remaining seated although I have provided him with the opportunity to get up during class and pass out papers. I am going to move his seat to the back of the room and allow him to stand as needed, provided he stays in the area behind his desk. If the behavior continues, I will refer my concern to the team leader for further support so that Jonathan can be as successful as possible. Thank you for your support."
Use this form to refer the student to the team leader. The team leader puts an intervention in place.
If the behavior improves, the teacher calls home to let the parent know and the teacher will continue the intervention with routine monitoring.
Example: "Jonathan has responded well to the intervention and I am pleased to report that he has been able to remain in his seat. I'm really pleased with his progress. Thank you for your support."
If the behavior does not improve, the teacher will call home to let the parent know and will complete an office referral to the grade level administrator.
Example: "Jonathan has not responded as we hoped he would to the team leader intervention. I will complete an office referral to the grade level administrator to request support so that Jonathan can meet with success. The administrator will contact you with further information. Thank you for your support."
Teacher makes referral to grade level administrator.
Teacher DOES NOT input administrator-managed behaviors in FOCUS
These behaviors immediately are brought to the attention of the administrator
Teacher emails the statement to the administrator - email should be sent by COB same day
Administrator puts the referral in FOCUS
