Process to Enroll in CHS:
Parent/Guardian must complete registration forms online

Once online forms are completed, see Ms. Queen in the attendance office at CHS to provide any additional required documentation. Enrollment documentation may include any/all of the following:
Birth Certificate
Picture ID
Immunization Record
Proof of Residency (2) (mortgage, lease, rental agreement, real estate property tax statement, current utility bill)
Name/Address/Phone Number of the Last School Attended
Transcript from the Last School Attended
IEP or 504 Plan (for Special Education Students)
Once a parent/guardian completes the enrollment process with CHS, a request for records will be sent to the withdrawing school. Upon receipt of records, students will be registered for class.

Process to Withdraw from CHS:
Parent/Guardian needs to fill out withdrawal paperwork in person at the CHS Counseling office with Ms. Darby
Must provide name of school that student is withdrawing to
Return school computer & charger
Pay any owed fees
Compelte enrollment process with school of choice
Once a parent/guardian completes the enrollment process with the school of choice, the school will send a request for records to CHS. Upon receiving a request for records, student information will be released to the requesting school.
