STEP 1 Requirements

Submit the following required documents via zoning@mandauecity.gov.ph: 

• • Sworn Declaration of Capital Investment (Application Form)

(Download a copy) For the appropriate PSIC code consult BPLO, or refer to the 2019 Updates to the 2009 PSIC)

• Certified and Updated Tax Declarations for Tax Map, Land and Building

(See the guide below for steps on how to acquire it)

• DTI/SEC (complete set of pages) /CDA

(For DTI: Apply DTI , For SEC: Apply SEC , For CDA: Apply CDA)

Your application is for verification and land use evaluation.

STEP 2 Requirements

Submit the following additional required documents via zoning@mandauecity.gov.ph: 

• Barangay Clearance

(Barangay Hall of where your business is/are located)

• Certificate of Occupancy / OBO Certification

(See the guide below for steps on how to acquire it)

• Contract of Lease / Affidavit of Consent 

(Acquire it through your lessor)

• • • Homeowner's Consent (if the property is located in a subdivision)

    • Legal Undertaking (for businesses such as internet services, beer/liquor sales, cigarette sales, and massage/spa establishments)

    • MCWD Bill (for laundry or water refilling stations)

Your application is for verification and processing

Requirements

• Letter Request

(Acquire it in Business Permit & Licensing Office (BPLO))

• Updated Special Power of Attorney(SPA) / Secretary's Certificate (if not the owner)

Process

• Submit your requirements to the City Assessor's Office

Note: You may also bring any of the following documents for easier reference of the City Assessor's Office when checking the details in the system.

• Contract of Lease / Affidavit of Consent / No Rental

• Tax Declaration Land / Building (issued prior this year)

• Real Property Tax Receipt
