Request for Certification/Document Portal

The Request for Certification/Document Portal serves as an alternative means of receiving applications from current and former students of the UST College of Architecture.

This Portal is in full compliance with the Data Privacy Act of 2012 in terms of use, storage, and disposal.

FOR GRADUATES
* PROSPECTUS (Summary of courses taken)

The applicant must send the following documents:

(1) Formal letter addressed to Ar. Rodolfo P. Ventura, Dean, UST College of Architecture indicating the following
(a) a proof that the course prospectus is requested/required;
(b) contact information in which we can submit this request directly;
(c) website link where we can verify that your requested documents are indicated for your purpose;

(2) and a scanned copy of your transcript of record (TOR).

FOR UNDERGRADUATE STUDENTS

* RESEARCH LETTER

The applicant must indicate the following details:

(a) course/subject for which the research is being undertaken,

(b) addressee (complete name, position/designation, address, and contact details),

(c) specific request details (e.g. to conduct an interview, request for documents like blueprints, etc.).


* CERTIFICATE OF GRADE

The applicant must indicate the following details:

       (a) specific academic year of his/her request.


* CERTIFICATE OF ENROLLMENT

The applicant must indicate the following details:

       (a) specific academic year of his/her request.

FOR BOTH GRADUATES AND UNDERGRADUATES

* COURSE DESCRIPTION

The applicant must indicate the following details:

(1) specific course/subject of his/her request,

(2) a scanned copy of your transcript of record (TOR) for graduates,

(3) or temporary transcript of records for undergraduates.


* GOOD MORAL CHARACTER CERTIFICATION

Procedures in requesting Good Moral Character Certification:

(1) Upload a clear scanned copy or photo of your UST school ID/Alumni ID and/or

latest Registration Form;

(2) and if the purpose of your application is for transfer, please state the name of the

college or the school.

* ACADEMIC HOURS

Procedures in requesting Academic Hours:

(1) Upload a clear scanned copy or photo of your UST school ID/Alumni ID,

(2) a copy of your Transcript of records.

(3) Please state the purpose of your request


DOCUMENT AND CERTIFICATE FEES:

      Academic Hours                   - ₱ 50.00   per page

Research Letter - ₱ 50.00   per page

Certificate of Grades - ₱ 50.00   per page

Good Moral Character - ₱ 50.00   per page

Certificate of Enrollment - ₱ 50.00   per page

Course Description - ₱ 50.00   per page

Prospectus - ₱ 1,000.00 per request

STEP 04 - Further Instructions, Other Reminders, Payment Procedures, and Agreement for Understanding the Process of this Request for Documents/Certificates Portal

AFTER THE SUBMISSION:
(1) Wait for the email from office.archi@ust.edu.ph for further instructions and requirements before you proceed to the Payment Verification Form.
(2) A transaction number will be sent via email by office.archi@ust.edu.ph. Upon receipt, you are now allowed to proceed for Payment Verification Portal HERE.
(3) The process takes within TEN (10) WORKING DAYS UPON PAYMENT VERIFICATION not including Saturday, Sunday, and/or Holiday in determining the due date of application.
(4) The release of all Documents/Certificates is only on FRIDAYS.
