OR COMPLEX AND PACU
A. JOB SUMMARY
The Nurse IV and III is responsible for supervising, implementing and evaluating quality nursing care among patients undergoing surgical procedures/operations and immediate post-operative care.
Coordinates health programs and continuing education activities among the staff. Ensures the smooth,
harmonious, interpersonal working relationship among the health personnel. Reports to Nurse VI all identified problems/concerns and significant events. The Nurse III relieves the Nurse IV in her absence. Acts as OIC on weekends/ holiday in the unit/clusters.
B. STATEMENT OF DUTIES
1. Patient Care
For OR Complex:
a. Allocates assignment of personnel, operating theater based on the type of surgical procedure/operation for 24 hours.
b. Directs the plan of care of all patients.
c. Allocates assignment of personnel based on patient’s needs and capability of the nurse.
d. Ensures implementation of ICC standards.
e. Ensures patients safety intra-operatively.
f. Collaborates with the other members of the health team.
g. Communicates significant findings appropriately.
h. Refers patient to the physician accordingly.
i. Checks /countersigns the completeness of documents.
j. Endorses/ handovers patients accordingly.
k. Attends to emergency resuscitative measures as needed.
For PACU (Post Anesthesia Care Unit):
a. Directs the plan of care for all immediate post-operative patients.
b. Allocates assignment of personnel based on patient’s needs and capability of the nurse.
c. Ensures the implementation of the nursing care plan based on nursing standards.
d. Ensures implementation of ICC standards.
e. Evaluates response of patients to nursing care rendered.
f. Collaborates with the other members of the health team.
g. Communicates significant findings appropriately.
h. Refers patient to the physician accordingly.
i. Checks /countersigns the completeness of documents.
j. Endorses/ handovers patients accordingly.
k. Performs direct nursing care.
l. Attends to code calls outside of the unit
m. Attends to emergency resuscitative measures as needed.
n. Provides psychosocial, emotional and spiritual support respecting their cultural beliefs and values.
o. Provides relevant health teachings/ health educational activities for patient’s,
families and significant others.
p. Ensures proper safe keeping of medical records.
2. Human Resource Management
a. Prepares work schedule and ensures proper distribution of nursing personnel.
b. Delagates work assignment among nursing personnel.
c. Assists the Nurse VI in all aspects of management functions.
d. Develops activities that promotes motivation and smooth interpersonal workin relationship in the unit
e. Counsels nursing personnel and refer accordingly.
f. Assigns/supervises work done by approved hospital volunteer personnel, students and trainees to the different clinical areas.
g. Evaluates performance of all nursing personnel.
h. Endorses to Nurse VI nursing personnel for promotion, transfer, resignation and termination.
3. Unit Management
a. Identifies needs for equipment, articles and supplies.
b. Sets control measures for the proper utilization of resources.
c. Documents unit management activities, incidents and related concerns.
d. Keeps in trust required records, reports and statistics for administrative purposes
e. Keeps an inventory of all articles, medical equipment, instruments and supplies.
f. Ensures compliance with established hospital policies, procedures, objectives and quality control.
g. Maintains a clean, orderly and safe environment conducive to patient’s recovery.
h. Signs in the Accountability Release of Equipment (ARE) of all articles, medical
equipment, instruments and supplies.
i. Ensures completeness and availability of narcotics and E-cart par levels.
(for PACU only).
4. Education and Training
a. Adopts programs that foster clinical development for career satisfaction.
b. Acts as resource person to the medical and nursing staff, students and trainees/observers.
5. Research
a. Identifies nursing problems requiring investigative studies.
b. Participates in the research process and implements results of studies which are
appropriate to patient care
6. Networking / Linkages
a. Maintains an open line of communication with the medical / paramedical and administrative personnel.
b. Participates in local and national health related projects and activities.
7. Miscellaneous
a. Accepts assigned responsibilities as delegated by the Nurse VI.

D. QUALIFICATIONS
Nurse IV:
• Education: Bachelor of Science in Nursing
• Experience: Two (2) years of relevant experience
• Training: Eight (8) hours of relevant training
• Eligibility: R.A. 1080
Nurse III:
• Education: Bachelor of Science in Nursing
• Experience: One (1) year of relevant experience
• Training: Four (4) hours of relevant training
• Eligibility: R.A. 1080
