You are ill.
Immediate family members are ill and require your care.
There is a death in your family.
You or your immediate family have scheduled medical appointments and are unable to schedule during your days or time off work.
If you have questions about your sick leave accrual and use, please contact your DiningHiring@ucsc.edu or your Manager.
Using the Missed Shift Form, you will also be able to designate your sick leave used as 'protected' for a qualifying reason.
Reasons include:
The diagnosis, care, or treatment of an existing physical or mental health condition of the employee or the employee’s family member;
Preventive care for an employee or an employee’s family member; or
Those reasons specified in policy for an employee who is a victim or whose family member is a victim of domestic violence, sexual assault, stalking, or other qualifying acts of violence.
If you become ill and cannot come to work, it is your responsibility to contact manager according to the missed shift procedures.

You may be asked to provide documentation to support your use of sick leave.
Whenever possible, schedule Doctor’s appointments before or after your shift or on your days off.
If it is not possible to schedule an appointment outside of your work schedule, you must get approval from your manager no less than 48 hours in advance of a scheduled absence. Appointments due to emergencies may be considered extenuating circumstances.
You may be required to furnish satisfactory proof (doctor’s note) of the appointment.
If you leave your employment prior to retirement from the University, your accrued sick leave will not be paid out to you.
Sick leave may be used for bereavement upon the death of a family member. Please refer to your current collective bargaining agreement or contact your manager or Employee and Labor Relations Analyst for more specific information.

The number shown under ENDING BALANCE is what has already been earned and is available to be used

Accrued sick leave is credited and available for use on the next working day following each month or quadriweekly pay cycle in which it is earned.
An eligible employee accrues sick leave based on hours on pay status. Sick leave is accrued at a factor of 0.046154 per hour (approximately eight hours of sick leave per month for an employee on full-time pay status). Eligible employees begin to accrue sick leave at the start of their appointments.
