Certificate issuance service at convenience stores 

A service is available to obtain certificates at convenience stores.  Two types of certificates that can be issued for graduates are "Certificate of Graduation (Completion)" and "Transcript of Academic Record," which can be obtained at multi-copy machines located at Seven-Eleven, FamilyMart, and Lawson stores nationwide after applying online.

For graduates/graduates before September 2012 (up to March 1970), only application and payment are available, and the payment will be mailed to you.

Issuance of Certificates after Graduation(Faculty of Life Design, Social Welfare, Health and Sport Sciences / Graduate School of Social Welfare Studies, Life Design, Welfare and Social Design, Health and Sport Sciences)

You can apply for various certificates at the Akabanedai Office (1) or (2) by mail. Applications by phone, fax, or e-mail will not be accepted.

※Online application and issuance at convenience stores are available for graduation certificates and transcripts, depending on the year of graduation.


Identification documents (both at the counter and by mail)

※We are experiencing a great number of incomplete identification documents. We are unable to issue certificates if the documents are incomplete.

 Please check the "Notes and Remarks" carefully regarding the certificate to be enclosed.

 

For the protection of personal information, identification documents must be presented (or a copy enclosed if applying by mail) to verify the identity of the claimant when the certificate is issued.

In addition, when an application is made by a person other than the applicant (hereinafter referred to as "proxy"), a letter of attorney and identification documents of the applicant (a copy is acceptable) and identification documents of the proxy must be presented (enclose a copy in the case of an application by mail).

If any of the above documents are missing or incomplete, we will not be able to issue a certificate. Please understand this beforehand.

※For those who have "changed" their last name since graduation

In addition to the originally required identification, one of the following is required (not required for identification of a proxy) For a certificate of residence and an extract from the family register (certified copy), please submit one issued within the last 3 months.

  ※1 Please refer to this page of the Ministry of Internal Affairs and Communications (MIC) for the "About the use of the former name on the resident card and the personal number card".
  ※2 Please refer to this page of the National Police Agency for information on "Concurrent Use of Former Surname on Driver's License".
 

However, the certificate will be issued in the name used at the time of graduation (not in the name used at the time of application).

If the applicant has "changed his/her name" since graduation, an "abstract (copy) of family register (original, not a photocopy)" is required in addition to the originally required identification (not required for a proxy's identification). As for the certificate of residence and extract (transcript) of family register, please submit the one issued within the last 3 months. However, certificates will be issued in the name of the applicant at the time of graduation (not in the name of the applicant at the time of application).

Handling certificates (including handling fees and number of days for issuance) (for both over the counter and by mail)

※Proof of enrollment for those who have left the school (including expulsion) is a "Certificate of Period of Enrollment" (500 yen).

※For certificates of academic ability, the fee is 500 yen per subject for each type of license.

※Fees are per certificate and are current as of April 1, 2023. For certificates other than those listed above, please contact the relevant office.

Number of days to issue

 The number of days for certificate issuance is as follows. Please apply well in advance.

※Certificate of Graduation (in Japanese)" and "Transcript of Academic Record (in Japanese)": Same day "Other certificates": 7 business days  (excluding Sundays, national holidays, and other university holidays)

※In the case of an application by mail, the number of days required for mailing will be added. *Please note that it takes a few days to issue  certificates during holidays, such as long vacation periods and university festivals.

Please print out the prescribed application form from the automatic certificate issuing machine at the Akabanedai Campus, fill in the required information, and submit it to the Akabanedai Administrative Section together with the following documents.
