Graduate School - Issuance of Various Certificates
Certificate issuance service at convenience storesA service is available to obtain certificates at convenience stores.Two types of certificates that can be issued for graduates are "Certificate of Graduation (Completion)" and "Transcript of Academic Record," which can be obtained at multi-copy machines located at Seven-Eleven, FamilyMart, and Lawson stores nationwide after applying online.
※For graduates/graduates before September 2012 (up to March 1970), only application and payment are available, and the payment will be mailed to you.

Issuance of Certificates after Completion of Course
You can apply for various certificates at convenience stores, in person at the relevant counter at each campus, or by mail. Requests cannot be made by phone, fax, or e-mail.
To protect your personal information, you will be required to show proof of identification (or enclose a photocopy if applying by mail) when the certificate is issued.
If a person other than the applicant (hereinafter referred to as "proxy") applies for the certificate, a letter of attorney and identification documents of the applicant (a photocopy is acceptable) and identification documents of the proxy must be presented (enclose a photocopy if applying by mail). Without the above documents, a certificate cannot be issued.
The number of days for issuing certificates is as follows Certificate of Completion (in Japanese)" and "Transcript of Academic Record (in Japanese)" in the format of the university: Same day (Please come to the counter by 30 minutes before the counter closes). Certificates other than those listed above: About one week Please allow enough time for your application (excluding the time required for mailing if you apply by mail). *Please note that the number of days until issuance may differ during long vacation periods and university festival periods.
If there is a designated form for certificates, please be sure to bring it with you (or enclose it if applying by mail).
For certificates related to academic ability, please specify whether the new law, the old law, or both the old and new laws apply, clearly state the subject matter to be certified (e.g. middle school social studies, high school civics, etc.), and enclose a copy (front and back) of your basic driver's license.
If you wish to have the certificate strictly enclosed, please inform the respective office (or clearly indicate this when applying by mail).
Personal information presented for identification purposes will be registered in the graduate data with the consent of the applicant. We will make every effort to properly manage and protect the alumni data, and will use it only to the extent necessary for Toyo University Educational Corporation to contact you, and will not use it for any other purpose. The submitted identification documents will be destroyed responsibly after issuance of the certificate.
About identification documents
Please prepare one of the following identification documents. (If applying by mail, please enclose a copy)
・Driver's license
・Passport
・Health insurance card
・Personal number card
・Basic Resident Registration Card
・Residence card
※ID documents must be presented each time an application is made (a copy must be enclosed if applying by mail).
※If you use your health insurance card as your identification document (both at the counter and by mail), please mask out (1) the symbol, (2) the number, and (3) the insurer number on the insurer card.
※If you are mailing a photocopy of your Personal Number Card as your ID document, please send a photocopy of only the front side of the card, as the back side contains your My Number.
※If you have "changed your family name" since graduation, one of the following is required in addition to the originally required identification (not required for identification of a proxy). For a certificate of residence and an extract from the family register (transcript), please submit one issued within the last three months.
A copy of the abstract of family register (original, not a photocopy) with maiden name (former name)
Certificate of residence with maiden name (original, not photocopies)※1
Personal identification number card with maiden name (maiden name)※1
Driver's license with maiden name (former name) written on it※2

However, the certificate will be issued in the name used at the time of graduation (not in the name used at the time of application).
※If the applicant has "changed his/her name" since graduation, an "abstract (copy) of family register (original, not a photocopy)" is required in addition to the originally required identification (not required for a proxy's identification). As for the certificate of residence and extract (transcript) of family register, please submit the one issued within the last 3 months. However, certificates will be issued in the name of the applicant at the time of graduation (not in the name of the applicant at the time of application).

Please print out the prescribed application form from a certificate issuing machine on each campus, fill in the required information, and submit it together with the following documents to the appropriate counter.

※If you require an English completion certificate, an English academic transcript, or an English certificate of credit withdrawal, please indicate your name in English (as it appears in your passport), where it should be submitted, and for what purpose.
※Fees are per certificate and are current as of April 1, 2023. For certificates other than those listed above, please contact the appropriate office.
※Please inquire about the amount for certificates on forms other than the University's form. *Credit-course students' certificates (in Japanese) cost a flat fee of 500 yen.
※A flat fee of 500 yen per subject is charged for certificates related to academic ability when the new law or old law is applied, and 500 yen per subject when the old law is applied, regardless of the number of subjects (however, a separate 500 yen fee is required for certificates for nursing care [special support]).
