Issuance of graduate certificates (Commuter courses)
Certificate issuance service at convenience stores
A service is available to obtain certificates at convenience stores. Two types of certificates that can be issued for graduates are "Certificate of Graduation (Completion)" and "Transcript of Academic Record," which can be obtained at multi-copy machines located at Seven-Eleven, FamilyMart, and Lawson stores nationwide after applying online.
※For graduates/graduates before September 2012 (up to March 1970), only application and payment are available, and the payment will be mailed to you.

Issuance of certificates over the counter or by mail
Please apply for various certificates by mail. Applications by telephone, fax, or e-mail are not accepted.
To protect your personal information, you will be required to present identification documents (enclose a copy if applying by mail) to prove that you are the applicant when the certificate is issued.
If a person other than the applicant (hereinafter referred to as "proxy") applies for the certificate, a letter of attorney and identification documents of the applicant (a photocopy is acceptable) and identification documents of the proxy must be presented (a photocopy must be enclosed if the application is made by mail). Without the above documents, the certificate cannot be issued.
About identification documents
Please have one of the following identification documents available
・Driver's license
・Passport
・Health insurance card
・Personal number card
・Basic Resident Registration Card
・Residence card
※Identification documents must be presented each time you apply (or enclose a copy if applying by mail).
※If you use your health insurance card as your identification document (both at the counter and by mail), please mask (1) the symbol, (2) the number, and (3) the insurer number on the insurer card.
※When sending a photocopy of the personal number card as identification documents, please send a photocopy of only the front side of the card, as the back side contains my number.
※If you have "changed your family name" since graduation, one of the following is required in addition to the originally required identification (not required for a proxy's identification). For a certificate of residence and an extract from the family register (transcript), please submit one issued within the last 3 months.
A copy of the abstract of family register (original, not a photocopy) with maiden name (former name)
Certificate of residence with maiden name (original, not photocopies)※1
Personal identification number card with maiden name (maiden name)※1
Driver's license with maiden name (former name) written on it※2
※1 Please refer to this page of the Ministry of Internal Affairs and Communications (MIC) for the "About the use of the former name on the resident card and the personal number card".
※2 Please refer to this page of the National Police Agency for information on "Concurrent Use of Former Surname on Driver's License".
However, the certificate will be issued in the name used at the time of graduation (not in the name used at the time of application).
※If the applicant has "changed his/her name" since graduation, an "abstract (copy) of family register (original, not a photocopy)" is required in addition to the originally required identification (not required for a proxy's identification). As for the certificate of residence and extract (transcript) of family register, please submit the one issued within the last 3 months. However, certificates will be issued in the name of the applicant at the time of graduation (not in the name of the applicant at the time of application).
1.How to apply at the counter(Hakusan Campus)
Please print out the prescribed application form from the certificate issuing machine on Hakusan Campus, fill in the required information, and submit it together with the following documents to the counter of the Educational Affairs Section of each faculty.

※Fees are per certificate and are current as of April 1, 2021. For certificates other than those listed above, please inquire at the Educational Records and Programs Section of each faculty.
※For graduation certificates, academic transcripts, and character certificates other than those on the University's forms, the fee is 500 yen.
※Certificates of Credited Auditors (in Japanese) are 500 yen each.
※Certificate of enrollment for those who have withdrawn from the university (including those who have been expelled from the university) is "Certificate of the period of enrollment" (500 yen).
※For "Certificate of Academic Proficiency", 500 yen per subject if the new law or old law is applied, or a flat fee of 500 yen regardless of the number of subjects if the old law or old law is applied (however, an additional 500 yen is required for the certification fee for nursing care [special support]).
4.Number of days to issue
The number of days for certificate issuance is as follows. Please apply well in advance.
"Certificates of Graduation (Japanese)" and "Transcripts (Japanese)": Same day
"Certificates other than the above": About 1 week
※If you apply by mail, the number of days required for mailing will be added.
※Please note that the number of days required for issuance may differ during long vacation periods and university festivals.
5.Other
If there is a designated form for certificates, please be sure to bring it with you (or enclose it if applying by mail).
The "Certificate of Character" can be issued only to those who have submitted the "Research Report on Character" at the time of enrollment. However, it is valid for three years after graduation.
For the "Certificate of Academic Proficiency," please specify whether the new law, the old law, or the two old laws apply, and the subject matter to be certified (e.g., middle school social studies, high school civics, etc.).
If you wish to have your certificates sealed in a sealed envelope provided by the university, we will seal your certificates in a sealed envelope and deliver them to you. If you wish, please indicate how you would like your certificates to be sealed (e.g., one certificate at a time, etc.). If you apply by mail, please specify in the designated space, if you apply at the counter, please tell us in person.
Personal information presented for identification purposes will be registered in the alumni data with the consent of the individual. We will make every effort to properly manage and protect the alumni data, and will use it only to the extent necessary for Toyo University Educational Corporation to contact you, and will not use it for any other purpose. The submitted identification documents will be destroyed responsibly after issuance of the certificate.
