Issuance of Certificates after GraduationCorrespondence education course

This section explains the procedures for issuing certificates for those who were enrolled in Toyo University's correspondence education course.

About the main certificates that can be issued

1.What you need to apply

Please send the following four items in an envelope to the address below (applications by phone, fax, or e-mail are not accepted).

(1)Toyo University Correspondence Course "Application for Issuance of Certificate (Application Form): Graduates and Others.
(2)Fixed-sum money order for the certificate fee (see (1) "Certificate Application (Application Form)" for each certificate fee). 

         ※Please check the amount and enclose it so that there will be no excess or deficiency.  ※Do not write anything on the money order.                                     (3)Copy of identification (driver's license, insurance card, etc.) For details, please refer to "5.
(4)Credit Sealer
   ・Write your name and address on a long 3/4-sheet envelope (※)
   ・Stamps: If the total weight of certificates is 1 or 2, it should not exceed 25 grams of letter (standard size mail); if 3 or 4, it should not     exceed 50 grams of letter (standard size mail).
   (※)For five or more letters, a Kakugata 2 envelope (23 cm in width and 31 cm in length) that can hold an A4-size letter and a stamp       of up to 100 g for non-standard size mail.

3.Number of days required to issue a certificate

It takes about one week from the time the application form is received to issue the certificate (excluding the mailing period and the period when the Correspondence Office is closed). Please note that certificates cannot be issued on the same day even if you apply in person at the Distance Learning Division.

4.Precautions, etc.

◆ For the protection of personal information, the applicant must present (or enclose a copy of if applying by mail) an identification document   that verifies the applicant's identity when the certificate is issued.

◆ Please note that if any of the required documents are missing or incomplete, we may not be able to issue the certificate or may delay its   issuance.

5.About identification documents

 Please prepare one of the following identification documents (this document for over-the-counter applications, or a photocopy for  applications by mail).

※If you have changed your name or surname from the one you used when you were enrolled in school, you will need to submit a copy of your  family register (original, not a photocopy) in addition to the required ID (in the case of a family name change, a personal number card,  resident card (original, not a photocopy) or driver's license with your maiden name on it is also acceptable). For the certificate of residence  and extract from the family register (transcript), please submit one issued within the last three months. However, the certificate must be  issued in the name of the applicant at the time of enrollment (not in the name of the applicant at the time of application). The ID of the proxy  is not required. 

※The identification document must be presented for each application (enclose a photocopy if applying by mail).

6.When the application is made by a proxy

When an application is made by a person other than the applicant (hereinafter referred to as "proxy"), a letter of attorney [PDF file/113KB] and identification documents of the applicant (a copy is acceptable) and the proxy's identification document must be presented (a copy must be enclosed for applications submitted by mail), in addition to the above "1.
