Certificate issuance service at convenience stores

A service is available to obtain certificates at convenience stores. Two types of certificates that can be issued for graduates are "Certificate of Graduation (Completion)" and "Transcript of Academic Record," which can be obtained at multi-copy machines located at Seven-Eleven, FamilyMart, and Lawson stores nationwide after applying online.

For graduates/graduates before September 2012 (up to March 1970), only application and payment are available, and the payment will be mailed to you.

Issuance of Certificates after Graduation

Applications for various Junior College certificates cannot be accepted by telephone, fax, or e-mail. To protect your personal information, you will be required to present identification documents (enclose a copy if applying by mail) to verify your identity when requesting a certificate. In addition, when an application is made by a person other than the applicant (hereinafter referred to as "proxy"), a letter of attorney and identification documents of the applicant (a photocopy is acceptable) and identification documents of the proxy must be presented (enclose a photocopy in case of application by mail). Please note that a certificate may not be issued if any of the following application documents are missing, incomplete, or both.

 

About identification documents

 For identification documents (applicant and proxy), please have one of the following certificates ready (enclose a copy if mailed).

【If you changed your family name after graduation】

・The certificate will be issued in the name of the student at the time of enrollment (the current name cannot be issued).

・Please present your ID document with each application (or enclose it if you send it by mail).

・Please inquire at the respective issuing (or renewal) office regarding the procedure for adding your maiden name to your current  name.

1.How to apply at the counter

 Please print out the prescribed application form from the certificate issuing machine on the Hakusan Campus, fill in the required information, and submit it together with the following documents to the General Affairs Section of the General Affairs Department.

※For Saturdays, applications for certificates at the counter must be made by appointment. Please contact the General Affairs  Section of the General Affairs Department in advance.

2.How to apply by mail

 Please send the following documents to the General Affairs Section of the General Affairs Department by mail.

 Please note that if any of the application details or documents are missing, the application will be issued only after the details and documents are  complete.

※2 For certificates other than those listed above, please contact the following office. 

※3 Certificates for non-degree students (in Japanese, in the format of the University) will be issued at a flat  rate of 500 yen. 

※4. Due to a notice from the Ministry of Education, Culture, Sports, Science and Technology (MEXT), the  University does not issue certificates for librarianship

If you need to clarify your qualifications as a librarian, please apply for the following two certificates

※5The content and format of the Certificate of Academic Proficiency differs depending on where the    application is made, and the applicable law and subject matter must be stated.

※6We do not issue "Certificate of Enrollment and Period of Enrollment" to graduates.

 ★Graduates who need a certificate of the period they were in school or enrolled in the program for pension procedures, etc., should apply for a "Certificate of Graduation".
