SYMPTOM SCREENING CHECKLIST

Home Health Assessment (prior to coming to school)

Thank you for partnering with the South-Western City School District to help keep our students and staff members safe and healthy.

  1. If your child has had close contact (within 6 feet for at least 15 minutes and not wearing a mask) in the last 14 days with anyone who has tested positive for COVID-19 or if any health department or primary health care provider has been in contact with you and advised you to quarantine, please do not send your child to school.

  2. Every morning as you’re helping your child get ready for school, please take their temperature. If they have a fever of 100.4°F or above, please keep them home.

  3. Please also check your child(ren) for any possible symptoms of COVID-19. People with COVID-19 have had a wide range of symptoms reported from mild symptoms to severe illness.

Symptoms may appear 2-14 days after exposure to the virus.

As outlined by Franklin County Public Health (8/25/2020), keep children at home if they exhibit:

Any of these COVID-19 symptoms:

  • Fever of 100.4°F or higher

  • New or worsening cough

  • Loss of taste or smell

  • Sore throat

  • Nasal congestion or runny nose

OR at least two of these COVID-19 symptoms:

  • Headache

  • Body aches

  • New or worsened fatigue

  • Nausea, vomiting or diarrhea

NOTE: This excludes any symptoms attributable to an alternative diagnosis.

If your child arrives at school with these symptoms or exhibits any of these symptoms during the school day, they will be assessed by a clinic staff member and we may call you and ask you to come pick your student up. Please follow up with your primary health care provider or Franklin County Public Health for further instructions.

In-School Screening Questions

Every day your child is physically in school, they will be asked these four screening questions:

  1. Have you had close contact (within 6 feet for at least 15 minutes cumulative) in the last 14 days with anyone who has tested positive for COVID-19 or has any health department or primary health care provider been in contact with you and advised you to quarantine?

Yes> The student should not be at school. Parents will be called and asked to pick their child up from school.

No> Thestudent can be at school if they are not experiencing any symptoms.

  1. Since you were last at school, have you had any of these symptoms?

  • Fever of 100.4°F or higher

  • New or worsened cough

  • Loss of taste or smell

  • Sore throat

  • Nasal congestion or runny nose

  • Headache

  • Body aches

  • New or worsened fatigue

  • Nausea, vomiting or diarrhea

  1. Did you take your temperature today and was it below 100.4°F?

Yes> The student can be at school.

No> The student's temperature will be taken to confirm it is below 100.4°F

  1. Since you were last at school, have you tested positive for COVID-19?

Yes or No

If a student answers 'yes' to questions 1, 2, or 4, a clinic staff member will conduct an assessment and we may call you and ask you to come pick your student up. Please follow up with your primary health care provider or Franklin County Public Health for further instructions.
