After completing the slip, you will fill out your sizes and information so it can be logged in the electronic system. Please take your time when doing this and make sure that it is accurate.

If you're in need of a uniform item, complete this form and you will be notified (by number or email) when to pick it up. Your request will be processed within 24 hours. You have to pick it up within 48 hours of being notified- or else it will be put back. We are not authorized to issue out gear the day of an inspection!
