Please read the information below prior to calling Cogdell Family Clinic as we are continuing to care for our community members, we appreciate your understanding.

This questionnaire and the answers you provide will be sent to an email that is handled by the medical and nursing staff.

Please allow 2-5 business days for a return call by our nursing staff. We will set up an appointment to schedule you for vaccination.

If you see an Oncologist or Rheumatologist, please contact their office for instructions regarding receiving the vaccination.

Please be advised that you will need to stay at the location for 15-30 minutes after you receive the vaccination.

This vaccine requires a second dose. The second dose will be due in 3-4 weeks after your first dose. Please plan accordingly prior to receiving your first dose.
