In accordance with Governor Abbott’s executive order GA-18, any Texas business choosing to operate, are required to follow minimum recommended health protocols. Employers may adopt additional protocols consistent with their specific needs and circumstances to help protect the health and safety of all employees and customers.
To help you in this transition, Cogdell Memorial Hospital is providing the following documents which can be used or adapted as you wish.
Included below are:
1. Minimum Standard Health Protocols provided by Governor Abbott
2. Screening log that can be used for the employee screening process
3. Self-Monitoring Guidelines to give to employees sent home, due to illness or symptoms
4. 14-Day Symptom Monitoring Log for the employee to use to document symptoms.
5. Prevent the spread of COVID-19 if you are sick document provided by the CDC as a reference.

The forms below will help you monitor your employees daily prior to work. Should an employee need to self monitor, you will find a form to send home with them.
