Staffing Updates
Emily's last day was 11/12/18
Sarah is going to be out for at least 6 weeks.
PRN Medications
Make sure we are filling the minimum amount on PRNs. At most I would fill 30 tablets, unless they need a dose during workshop hours in which case I would send 2 cards of 30.
Drug Allergies
Make sure when we admit a client that we are getting the drug allergies. If we can't get them make sure that we notate our effort to obtain them. When we enter orders for someone who has no drug allergies listed, we have to make sure to get them before we fill anything. I know it is unlikely that someone would be on something they have an allergy to, but there is a good chance that we may not know if they have actually been taking all the meds that were sent to us.
Faxing/Emailing Scripts
Make sure that we are faxing or emailing scripts to the list of facilities that have requested it. We have had a lot of instances where scripts are being sent to the Pending OE queue without being faxed/emailed to the facility first.
Quantity/Day Supply
We need to make sure that we are double checking the quantity and day supply on anything we fill/refill. We also need to make sure that if we are sending multiple bottles/packages/etc... that we are putting it in for the right day supply. If one bottle of eye drops is a 25 day supply and the facility wants two, we have to bill it as a 50 day supply. If it doesn't pay for that day supply, we cannot adjust the day supply to get it to go through. That is fraud. If the insurance has a limit of 31 days and two inhalers end up as a 32 day supply, they will have to pay cash for a second one or wait until insurance pays.
If we have an order for something that will have to be discarded before they run out, like an insulin, please make sure we are putting it in for the maximum day supply that the product is good for. A lot of insulin products are only good for 28 or 30 days, so we have to put them in for an appropriate day supply. Please see Insulin Storage and Stability for more info!
Refill Too Soon notifications
Please make sure that we are associating the refill too soon notifications to the actual medications. I am finding a lot of instances where someone says they requested a medication a few days ago and I have to search for every document ever associated to the profile to find the note in RTS. Associating to the actual drug makes it a whole lot easier to find.
Make sure we are putting detailed notes on them so we know how much needs to be sent on the day it will pay. Notes like "send for increased dose" are not sufficient, the person refilling it needs to know exactly how many and for what day supply to fill the medication. This is causing the person in reorders to do unnecessary research when we could have put a quick note in the beginning.
When we are filling something that is too soon until a future date but we are shipping the item, like on Friday when something will pay on Sunday, please make sure that you are changing the dispensed date to the day it will pay. I spend a lot of time having to look at rejections and flip dates on things that could have already been changed.
