Prior Authorizations

Please make sure that we are sending a 16 day supply on all PAs for affiliates, with the following exceptions.

  • • Contact Carrie Schryer and fill for the initial 4 day supply if insurance allows it. If it is a refill and we have done the 4 days they don't want us to send any more.

• 212/213/214

  • • Always ask for PTB from Tiffany Moore

• 243

  • • Always ask for PTB from Jacqueline Jones

• 410

  • • Always ask for PTB from Melissa Coleman

• 448

  • • Always ask for PTB from Wendy Adkins and Paula Cowan

  • • Contact the Guardian and the facility first. If nobody says “Don’t send it” then we need to. We just like to give them the opportunity to say no first.

All other Affiliates: Make sure to contact and let them know first. I like to give them a chance to say no, but if I don’t get a “no” by the end of the day I always send it. If it is >$500 I usually give them a little more time before I just send it out, or will try a little harder to get an answer from them.

Non-Affiliates: Do not send short fills without approval from them. I don’t offer a 16 day supply to non-affiliates, I will just give them a price per day and let them decide how many days to do. To figure the price per day I just give them the cost for a 1 day supply, and they can figure it out from there. It isn’t 100% accurate to the actual price, but it is better to over quote something than to under quote it.
