Creating & Using Google Questionnaires

Time: 4:00pm - 6:30pm at the Career Center

We will Create Questionnaires using Google Forms. Using Forms significantly reduces your work-load. Learning to be efficient is fun and empowering. We are going to dedicate this class to using Google's tools to collect data, eliminate data entry, and generate visual reports with graphs. I will teach you how to setup your form to email you alerts as questionnaires are being filled in. You will be introduced to the growing options of "Ad-Ons" to create Thank-You Letters, Certificates of Completion, Inventory Reports and Form Receipts. This is a great tool for Teachers, Parents, Business Owners and Non-Profit groups that need to learn to do more in less time.

Users should have a G-Mail account, be familiar with eMail, Word Processing, and Spreadsheets concepts. Many of you may know it is possible, and now you can learn to create these information gathering tools.

No software is required to be purchased, you can can bring your own laptops, tablets and phones to class or a desktop will be provided. If you have questions or would like help setting up a G-Mail account please call JilMac 802-254-8628 before class.

Forms are extremely versatile. The uses of forms and form add-ons grows every week. You can apply forms to an ever growing applications, as you learn to do more - you learn you can do more!

Forms can be used for a variety of things:

1. To gather data

2. To enter Inventory

3. To give a Test

4. To create a Training Tutorial

5. Allow only specific email addresses to enter data

7. Be copied from year to year

Forms can Include:

1. Images

2. Videos

3. Verifications for email or website addresses

4. Restrict only one entry per computer or email address

5. Radio Buttons so only 1 selection can be made

6. Check Boxes so multiple selections can be made

7. Pull down selections

8. Ranges and Grids

During or After the Data is collected you can have the Google Sheet (similar to a Spreadsheet)

1. Alert you that Data has been entered

2. Allow a user to review and change their selections

3. Display a Thank you page

Forms can be expanded or added onto:

1. Generate a Confirmation email or Certificate

2. Show a different page depending on a previous answer

Data from a form can be saved indefinitely and can be used for as long as you would like

Similar products such as: Survey Monkey have restrictions as to how many entries or how long data is saved.

What you can't do (yet)

1. Modify the data and expect the reports to update

2. You can not combine data from different forms and expect the reports to update.

Forms can be done in several ways.

    • The 1st is an attached document that the visitor to your site can print and mail in.

    • Click Insert > Documents > Select the Document you want linkable

    • Note: the Document that you are putting on the web site has to be SHARE > Public on the Web

    • 1. You can just add the LINK (you get this from your Documents-Share-Link to Share)

    • The 2nd - is an interactive form that you have the visitor to your site enter on-Line

    • Go to Google Docs/Drive

    • Create a NEW Document > Select FORM > enter the Form Information

    • Edit the page at the location you want the Electronic Form to display

    • Click Insert > Spreadsheet Form > Select the form
