FORMS FOR FINAL DEFENSE
Application for Thesis / Feasibility Study / Capstone Project / Research Protocol (F-RDO-006)
This form is used by the researchers in a research program to apply for a thesis, feasibility study, capstone project, or a research protocol to be approved by the Adviser, the Program Chair, the Dean, and the Research and Development Officer.
Application for Thesis / Feasibility Study / Capstone Project / Research Protocol (F-REC-001)
This form is used by the researchers in a research program to apply for a thesis, feasibility study, capstone project, or a research protocol to be approved by the Research Ethics Committee Chair and Member from the respective college or department.
Research Proposal / Protocol Template (F-REC-002)
This form summarizes the background information, the related literature, and the methods to be used in the research study. This also includes the work plan schedule, ethics clearance, research utilization, budgetary requirements, and the curriculum vitae of the researchers.
Research Application for REC Review (F-REC-003)
This form is used to apply a research protocol based on the type of study and the source of funding the researchers will be conducting. It includes the brief description of the study (i.e., abstract, executive summary).
Protocol Assessment Form (F-REC-004)
This form is used to check if there is a need for Research Ethics review and if the condition of the respondents may affect or harm the research study.
Informed Consent to Participate in Research (F-REC-005)
This form is used to provide human subjects (18 years old and above) with the information they need to decide for a research study without any coercion, undue influence or inducement, or intimidation.
Assent to Participate in Research (F-REC-006)
This form is used to authorize participation of research for human subjects (17 years old and below) who lack the capacity to give informed consent. It requires an honest and cooperative agreement among the researchers, the minor participant, and the parent or legal guardian.
This form is used to show a progress or update that the group is making towards completing a research project.
This form allows researchers to provide details of the completed research project.
Suspected Unexpected Serious Adverse Reactions / Serious Adverse Events Report (F-REC-010)
(Optional) This form assesses if the involved participants may pose harmful or insignificant risks to the research study. It would only be required if a participant is one of the following: prisoners, children below 17 years old, persons with disabilities, people with history of drug, sexual, or any forms of abuse, or if the research study involves the use of drugs, animals, and/or other medical devices.
