• You are only allowed to request 1 change per class

• Only fill out the form if you want to request a change

• You are not allowed to change Phys Ed or Health

• Filling out the form

  • Open the form → Type your Name

  • Select the Period/Day of your request

  • Select your CURRENT class

  • Select your DESIRED request

  • Fill out the form again for each different Period/Day

• This is a request, not a guarantee. All changes will be reviewed by admin
