IFM Community Connections Health Clinic
The Community Connections Health Clinic (CCHC) opened in 2023, and was made in partnership between IFM Community Medicine and the Connections for Health and Healing Program and SSM Health DePaul Hospital.
CCHC is open Monday morning and afternoon, Tuesday morning, and Friday morning. The clinic serves both the students of the Pattonville School District, but also the greater community effected by Bridgeton Landfill. The clinic serves residents of the 63042, 63043, 63044, 63045, and 63074 zip codes at no cost to the patients.
We are also open to serve the greater St. Louis area to help those with unaddressed health issues due to living in under resourced communities. CCHC provides a variety of services from sick visits and physicals to well woman visits and preventative care. Our medical professionals and Community Health Workers who staff the clinic work very hard to make sure the patients receive the medical care they need and deserve at the lowest cost possible for them.
Grace Hill provides primary and preventive health care through five community health center locations, primarily to uninsured and underserved residents of St. Louis and surrounding communities. GHNHC is accredited through The Joint Commission.

Healthy Kids Express performs screenings for hearing and vision, anemia, lead, dental, asthma and scoliosis. Services also include height/weight measurements and immunizations. The program was developed to bring health care to children from 1-18 years old in St. Louis city, surrounding counties and the Metro East. Healthy Kids Express is a site-based program, partnering with local school districts, Head Start programs, local day care centers, and other community sites to offer services. Healthy Kids Express social workers gather resources and are able to connect families to community resources such as low-cost health insurance and healthcare providers.

The Asthma Clinic for children operates from 8:30 a.m. – 12:30 p.m. on the third Friday of every month.
Dental Services
Please note that effective December 1, 2016, all Saint Louis County Department of Public Health dental visits are being scheduled at the John C. Murphy Health Center ONLY. Services are available for patients age 4 and up and include exams, x-rays, cleanings, dental sealants, fluoride treatments, fillings, and simple extractions. Hours are: 8:30 a.m.-12:00 p.m. and 12:30 p.m.-4:00 p.m. Please call 314-615-0530 to make an appointment.
Please also note that dental emergencies are handled according to severity and the ability of the clinic to accommodate and providers to effectively treat the emergency. Within these guidelines, walk-in emergencies are limited to patients with true emergencies.
