5 Essentials Survey

The 5 Essentials Survey is a brief survey that gives us valuable feedback and becomes part of our school's Interactive State Report Card. Parents may take the survey one time for each child they have in the district. The survey may take approximately 10 minutes and is open between February 3rd and March 13th.
