Qualtrics is a user-friendly, comprehensive electronic survey tool available for use by all Nazareth faculty, staff, and students. Qualtrics eliminates the need for the many survey design solutions that individuals have had to rely on in the past.

The system provides users with:

• A variety of question types

• Survey management tools

• Automated email distribution

• Customized data visualizations

• Reporting in many formats (.pdf, .ppt, URL, .doc, etc.)

Student Account Information

NEW! 4/18/25: All first-time log-ins are automatically Survey Creator accounts. Accounts created before this date but that were not upgraded will need to request an upgrade to Survey Creator using this form.

You must log in to the Nazareth University Qualtrics Site using your current MyNaz credentials (ex. asmith1, NOT @mail.naz.edu).
