Code of Conduct / Decorum / Expectations for Students

All students at KVA are subject to the Code of Conduct, Decorum, and Expectations policy. 

Permission Form (leaving campus, rides and transportation, and photographs)

Any student that leaves KVA for any reason should have a signed copy of this form submitted to KVA Administration.  If there is not a completed and signed form on file that a student, permission will have to be given on a case-by-case basis.

Allergy Awareness Form

From time-to-time people attend KVA who have allergies and/or sensitivities.  This form  is purely informational and only includes the type of allergy and how KVA staff can keep students safe.

Equipment Handling and Contact Form

KVA employs a variety of specialized and in some cases delicate equipment. Please review and sign the Equipment and Handling and Contact Form. It is a short review of appropriate handling and use of KVA equipment and facilities.

Photo/Video Waiver

This form gives permission for KVA and its agents to use photos and videos of students in marketing and educational materials.  If no form is on file the student will not be included in any pictures or videos used for any purpose other than KVA Administration (identification, transcripts, etc...)

Chemicals and Substances Awareness Form

KVA employs chemicals and substances in its Science programs as well as the Screen Printing program. Many of these chemicals require safe storage procedures as well as safe handling and use procedures. This form verifies that students and parents/guardians/agents are aware that these chemicals and substances are present at KVA and that thay may require safe handling and use procedures.
