Referrals Out

How to make referral out to another service:

Therapist actions: 

1.  Post in Slack to your Coordination channel, including the Client's Juvonno number

2. Ensure that the Patient's chart is updated with your notes

❓If unsure on whether the CYP should be referred out or not, post to your coordination channel and tag @Boyana for a case discussion / review. 

Coordination actions:  

1. First generate the appropriate referral form using the Documents > New Letter function in Juvonno (see screenshot below)

2. Send to the NHS CAMHS SPA team via sandbox@mindler.co.uk 

3. Update the Slack post to confirm that the task has been actioned

4. Update the CYP via text or email as appropriate 

⚠️Remember to check if the CYP was originally referred in by CAMHS, and/or if there is any other context that should be shared with the SPA team
