Outline the steps and guidelines for tracking CYP participating in internet-based Cognitive Behavioral Therapy (iCBT) treatment.
Ensure effective monitoring, assessment, and communication throughout the treatment process, in line with the needs of the individuals.
CYP will be assessed based on the severity of their self referral.. Those who are more severely impacted, or those who are younger, may require closer monitoring and potentially more frequent YP-core score checks. Coordinators should use their judgment based on the self-referral information and any additional referral data provided.
Once triaged, each CYP will be automatically assigned their first module, starting with "Emotional Recognition." Coordinators must verify weekly that automatic sign-ups and communications have been successfully uploaded/sent. In case of any failures in the automatic process, staff should investigate any potential errors from coordination input. Unresolved issues should be escalated to Amy to investigate further.
For undeliverable automatic text messages, coordinators will review profile formatting for errors. If resolution is not possible, coordinators will contact the CYP by email to acquire the correct phone number.
CYP are encouraged to notify the team when they complete their modules. Additionally, alerts may be received in the Sandbox email. Coordinators will review submitted modules, ensuring the absence of high-risk content such as mention of self-harm or suicide.
A text message will be sent to the CYP to inquire about their experience with the completed module. Answers from the module can be used to guide conversations and module assignments.Based on responses and the module content, the next appropriate module will be assigned. For instance, if the CYP frequently discusses anger or aggression in the emotions module, the anger module could be the logical next step, similarly coordinators can look to the presenting issue on the CYP’s profile and discussion within Juvonno messages.
YP-core scores will be sent out again after completion of four modules. However, if concerns arise about a specific CYP during this period, it can be conducted earlier. Monitoring will be ongoing to ensure that the treatment is aligned with the evolving needs of the CYP. (incomplete)
As CYP approaches the end of available modules, a check-in will be initiated to assess progress and discuss possible further support and resources. This process creates a gradual transition, akin to winding down in traditional therapy.
Upon completion of the iCBT program, a YP-core score assessment will be conducted.The graduation process will be followed, leading to discharge from the service.
CYP are allowed 8 weeks of non-engagement before discharge. This period begins either at sign-up or from last communication/active engagement on WorkRamp. If non-responsive, CYP should be contacted twice per month within the 8-week period, encouraging engagement with iCBT materials. Discharged individuals will be removed from the iCBT tracker, WorkRamp ( move to inactive), and marked as discharged in Juvonno.
