Need to change your address, phone number, tax withholdings or direct deposit?  You can do so online through ADP self-service for most efficient updates.  If you prefer to make changes on paper copy, download the forms and submit by having your supervisor scan to DOCS4HR on the copier.

For security purposes, direct deposit and change forms submitted via email from outside KTP will require additional verification prior to processing.  Direct deposit forms and employee personal info change forms are only available internally, on the KTPWEB company intranet, from your supervisor, or in paper format in the front office of your facility.
