Participate in Our Health Risk Assessment (HRA) Program for Medical Insurance Participants
Meridian Industries, Inc. has partnered with Integrated Health 21 (formerly Healics) to provide employees with Health Risk Assessments (HRA’s) again in 2024. HRA’s are available only to employees enrolled in Meridian’s medical plan.
*To qualify for the Wellness Discount (removal of tobacco surcharge), you must complete the HRA biometric screening and test negative for nicotine, or provide a nicotine test result at your own cost.
To get started, use the below instructions to register with Integrated Health 21 (IH21).
After your biometric screening you can log onto the IH21 wellness portal to see your results and access resources to help you on your personal wellness journey. You can also schedule a voluntary telephone consult with an IH21 coach to review your individual results.

Step 1 - Create Your Personal Online Account
It's easy to set up your secure online account, schedule an HRA appointment, and complete your Individual Health Profile Questionnaire.
To set up your account, go to www.ih21wellness.com
1. Click on, "New User? Register Here."
2. Enter your first name, last name, and birthday. These entries must match ADP and what is on your paycheck.
3. Use the company code: Meridian
4. Create your username and password.
5. Enter your email address, phone number, and submit.
If your account cannot be found, you can contact the iH21 customer service department at 800.451.6889 or via email at customerservice@integratedhealth21.com. Only employees on KTP medical insurance will be able to create an account.

Once logged into your account, on your dashboard, click on the button “Click to Complete” next to the Individual Health Profile activity. Complete the questionnaire prior to scheduling a screening date.
Step 3 - Register for an HRA Screening
Step 3 - Register for an HRA Screening
To schedule an appointment for an HRA screening, select “Click to Register” under Biometric Screening. Agree to the terms and conditions and follow the directions that follow.
Choose the screening location and date/time that works best for you, by clicking on “Choose Screening”, then select Onsite Screening. A general outline of dates and times for each location is listed below. If there is not a scheduled screening at your regular work site, please schedule for the next closest location.
If you are unable to make an appointment for your screening online, please contact HR for assistance at extension 6010 or via email at HR@kleentest.com.

The location of the HRA screenings at each facility will be determined by the number of participants (and thus, the amount of space needed). Screening locations will be posted before the date of the appointments.
Review these tips from IH21 to prepare for your screening appointment:
For the most accurate results, fasting of 8-12 hours is highly encouraged.
Decaffeinated black coffee or decaffeinated black tea will not alter results.
Drink plenty of water. Drinking at least 8-16 oz. of water 20 minutes prior to the screen is encouraged.
Take medications as prescribed by your doctor.
Wearing a loose fitting shirt will allow examiners to more easily measure blood pressure results.
If you need to reschedule your screening appointment, log in to your IH21 account and you can cancel or reschedule there.

Report: You will have access to your results through the online portal approximately two weeks after your biometric screening. Next to biometric screening, click on “View Your Numbers” to review your results. To review your Individual Health Profile (IHP) report, select “Click for Report.”
