New Hire Paperwork - 2 Easy Steps!
KTP HR will schedule you for an onboarding appointment upon completion of step 1.
Step 2 will be completed on site, during your onboarding appointment (scheduled with HR after you complete Step 1).
You will receive an email confirmation of your start date and time with details once scheduled to start work.
A pre-employment drug test may be randomly scheduled for new employees after you start work.
Step 1 of 2 - Kleen Test Products Electronic New Hire Paperwork (upon acceptance of offer)
Click the link above and enter the requested information to complete KTP's internal hire paperwork. Once you complete the form, a copy of the paperwork will be emailed to you and KTP HR. This step should take 5-10 minutes.
WHAT ID's DOES KTP NEED?
This list shows your options for providing us with the necessary ID's to complete your Form I-9. This must be completed before you are scheduled to start.
HR (or designated representative) will verify your identify and employment authorization based on these ID's at latest on your first day of work. Copies of your ID's will be kept on file with your Form I-9, confidentially and securely stored with Human Resources.
WHY DOES KTP NEED MY PHOTO?
On your first day, KTP will issue you a security badge (typically including a photo) for entrance into the facility and secured doors throughout your work area. Additionally, we will send a photo of you on the hire notice to your manager and supervisor, so everyone knows to expect you on your first day. For some positions, we may also use this photo for an employee announcement, or for other announcements throughout your employment.
IS MY CONFIDENTIAL INFORMATION SECURE?
Yes, your personal information is securely stored and records are deleted once everything has been entered into our systems.

Tips for Step 2 Mandatory Forms for New Hires
**You will complete these forms during an onboarding appointment with HR (or a designated representative) prior to being scheduled to start work.**
Form I-9 is required to be completed, fully and correctly for all employees, and this will be kept on file for the duration of your employment. HR will need the original copy of Page 1 with your signature, and copies of your provided ID's.
The federal W-4 form was revised in 2020 - if you are unsure how to complete this form, we suggest you refer to this information - https://www.irs.gov/individuals/tax-withholding-estimator
You will submit paper tax forms to accompany the electronic selections you made with your electronic acceptance paperwork. The elections you make on your paper forms will supersede your electronic responses.
Remember we need your original Form I-9 Page 1 in HR for record-keeping.
These forms are legally required for all hires and must be completed using the official copies of each form. You will be able to submit these on your first day of work, at the latest.
