NEW HIRE BENEFITS ENROLLMENT

FOLLOW THE STEPS BELOW to start your benefits enrollment. Action is required for all employees.

Elections made now will be effective for the remainder of the plan year, which begins on January 1st and ends on December 31st. You have the opportunity to change your elections every year during Open Enrollment, or if you have a Qualifying Life Event throughout the year.

Kleen Test Products offers two medical and dental insurance plan options, each with 4-tiers of coverage, and both through Anthem Blue Cross and Blue Shield. Our medical plans include prescription drug coverage plans, administered by Express Scripts. Vision insurance is administered through NVA. These insurances are not bundled, and the employee will select the specific coverage and tier to match their needs, for each type of coverage. An employee will become eligible for insurance coverage to start on the first of the month following 30 days of employment. Company-provided life insurance and company-provided disability insurance will be effective at no cost to the employee on the first of the month following 30 days of employment. Eligible employees will be automatically enrolled in a 401(k) account on the first of the month following 60 days of employment, administered by John Hancock Retirement Services, unless you contact John Hancock to opt out. 401(k) deferral rates will automatically increase every year by 1% until you reach 10%. Refer to the benefits booklet, plan documents, and benefits summary, for additional information on all benefits offerings and plan allowances and limitations. All of these documents are found HERE.

To ensure no delays, we ask that you please enroll within 7 days of your hire date or the date of your qualifying life event. Enrollment MUST be completed within 31 days of your hire date or the date of your qualifying life event.

Every employee MUST complete online enrollment or waiver before your coverage eligibility start date (1st of the month following 30 days of employment).

If you are already registered, you can skip this step.

If you are not already registered, watch this video and use the below links to help you get registered.

To create your account,

  • Select ‘Find Me’ to locate your record based on your contact information on file (phone/email)

  • If you are unable to locate yourself, contact HR to be issued a Personal Registration Code

3. Make Your Benefits Elections in ADP

4. Submit follow up forms or documentation to HR, if required.

Documentation should be submitted directly to benefits@kleentest.com or scanned to DOCS4HR from the facility main copier. NOTE - your enrollment may not be able to be completed if you do not submit all required documentation. Please double check this has been completed before the deadline!

Do any of the following apply to you?


Did you try to add a spouse or dependent to your plan? If so, submit required documentation to complete the addition. This may include a marriage certificate, birth certificate or other documentation depending on the specific circumstances.


Is your spouse on your plan? If so, submit B4 Spousal Coverage Statement. Failure to return the form means you will be subject to the surcharge, regardless of eligibility.


Are you tobacco-free and enrolled on our medical plan? If so, submit B3 Affidavit for Participation in the Tobacco-Free Incentive Plan. Failure to return the form means you will not be eligible for the discount, regardless of eligibility.

If you are unable to complete your open enrollment electronically due to technology or other limitations, you have the following two options:

1 - Use the shared onboarding computer station or another available computer at your facility (with your Supervisor's permission). Onboarding station computers are generally located in your facility lobby.

2 - Complete and submit paper forms, which can be downloaded HERE (scroll to the bottom of the page). Any submitted paper forms MUST BE RECEIVED BY HR by 5pm on the date of the deadline, and need to be scanned to DOCS4HR by your supervisor. Paper forms should not be put in the interoffice mail.

BE ADVISED if you are using a public or shared computer or device, you NEED TO LOG OUT of your personal ADP account when you are finished to maintain confidentiality of your personal information. ADP will automatically log you out after a period of inactivity, but don't rely on this feature to protect your privacy.
