DOMESTIC PARTNER INSURANCE
Insight PA offers health, vision, and dental insurance coverage to eligible domestic partners of Insight PA employees.
Qualifications:
Individuals are at least eighteen (18) years of age and mentally competent to consent to a contract.
Individuals are each other’s sole domestic partner and intend to remain so indefinitely.
Individuals are not married to or legally separated from anyone else.
Individuals are not anyone else’s domestic partner.
Individuals are not related by blood to a degree of closeness that would prohibit legal marriage in the state in which they reside.
Individuals are living together in the same residence and intend to do so indefinitely.
Individuals are engaged in a committed relationship of mutual caring and support and are jointly responsible for each other’s common welfare and living expenses.
Individuals must live together on a continuous basis for at least 12 months immediately prior to the date of the Declaration required by Insight PA.
Individuals are not in the relationship solely for the purpose of obtaining coverage.
Proof of Domestic Partnership - you must meet at least 3 of the following criteria:
Common ownership of real property (joint deed or mortgage agreement) or a common leasehold interest in property
Common ownership of a motor vehicle
Driver’s license or other state-issued identification listing a common address
Proof of joint bank accounts or credit accounts
Proof of designation as the primary beneficiary for life insurance or retirement benefits, or primary beneficiary designation under a partner’s will (if you have a life insurance policy through Insight, this counts! Contact HR if you need assistance obtaining proof of your beneficiary)
Assignment of a durable property power of attorney or health care power of attorney
Cost:
The premiums for including a domestic partner on your insurance are the same as if you were including a spouse - the rate is for a "couple". See the rates on page 12 of the benefits booklet.
While Insight PA doesn't charge more to insure a domestic partner, federal law requires that the portion of Insight's cost that goes towards insuring the domestic partner is taxable. As tax rates vary for every employee, there is no set rate. However, we have the ability to preview the taxes if you'd like to have an idea of what to expect for budgeting purposes.
Process:
Confirm that you meet all of the qualifications of domestic partnership and can provide 3 proofs of interdependency (above).
Read the Domestic Partnership Insurance Policy in full.
Complete the Declaration of Domestic Partnership Form.
Return the completed form to HR@insightpa.org.
