Hello Parent/Guardian,
Congratulations on successfully submitting your application, whether online or on paper. By doing so, you are now one step closer to joining our esteemed after-school program for the school year 2023-2024. We are thrilled about the possibility of having your child with us. My name is Sean Ash, and I'm excited to introduce myself as your Program Director. I'm here to guide you through the remaining steps to ensure full student enrollment and a smooth transition into our after-school program at Flatbush Development Corporation, along with the Assistant Director Avery Wilson.
Please follow the steps listed below to complete the enrollment process:
1. Online Student Application: If you've not already done this, please ensure it's completed.
2. Supplemental Student Application or DYCD Student Application: Please complete the relevant form.
3. Parent Orientation: Attend our session where we'll provide valuable insights into our program.
4. Student Behavior Policy Sign-Off: This is essential to ensure mutual understanding of our expectations.
5. Parent Handbook Policy Signature Page: Please review, understand, and sign off on our policies.
6. Confirmation Letter and Email: Once all the above steps are completed, you will receive an official confirmation of your child's start date.
It's crucial to note that student start dates are contingent upon the submission of all required paperwork as mentioned above.Should you have any questions or require further assistance, please don't hesitate to contact me. Registration for FDC After-School Program

If you do not have a DYCD Participant Account, please create one.
Once you have logged in into the Portal, select “Afterschool”
In the program, search: JHS 062 Ditmas
Select “add to list”
Select “add program” and select your child's name then click “apply”
Select dashboard and child's name then click “apply”
Once all information is completed you must attest and confirm "I am not a robot"
At the bottom click “submit”
Once submitted a box will appear stating "Success!..."
Shortly you will receive two emails from discoverDYCD@mail.dycd.nyc.gov. First email "discoverDYCD Application Submit Confirmation" & the second email "discoverDYCD Application Receipt Confirmation".
Once you have received your confirmation email from DYCD, you will receive an email confirming your child’s registration from FDC After School Program. We look forward to seeing you in September!
