The primary goal of this program is to help elementary and middle school students correct their academic trajectory early in their education when they begin to express behavioral difficulties in the classroom or show a need for enhanced social-emotional learning and coaching. At Henry County School of Scholars, we aim to achieve that goal within a supportive environment in which children are comfortable learning and growing, both academically and behaviorally.
Our certified teachers, special education staff, and therapists ensure students receive the highest quality of care in the least restrictive environment. Henry County School of Scholars is tailored to each student’s needs and designed to help our students excel academically, physically, functionally, socially, and emotionally. This learning environment is designed to:
Minimize triggers
Teach and model positive behaviors

Through Henry County School of Scholars, students improve their attendance and promotion rates, reduce behavioral incident rates, and demonstrate grade-level growth in both reading and math.

THE MENTAL HEALTH AND WELLNESS FACILITATOR'S ROLE
The Mental Health and Wellness Facilitator's role is to serve as the person in the school that submits the referral for the program.
The Facilitator will gather necessary information to complete the referral packet with help from the school staff including the teacher, administration, and SSF.
When the packet is completed, it will then be scanned and emailed to Dr. Debra DeLaine.
Referral Cover Sheet & Checklist

Classroom Behavior Observation Form
Some Helpful Tips:
Think about all the students who would benefit from this intervention. Not just students that are disruptive, but quiet students that are dealing with a lot of trauma.
Reason of Referral: This needs to be included and detailed. Your reason for referral should anser this question, "Why would this student benefit from School of Scholars?"
Work with your school team on pulling all the needed documents that you already have at school. DO NOT CREATE NEW DOCUMENTS. Follow the referral sheet for what should be included.
It is acceptable to include notes or synopsis.
If you include work samples, these must have anecdotal notes. Example: This was completed 2 hours with 14 redirections and the paper torn up 2 times.
Referral sheet, Student Summary Sheet, and the Behavior Survey need to be compete and included in the packet.
The student Behavior Survey is in this link: https://docs.google.com/forms/d/e/1FAIpQLSdAlzRDvkhrav27TWLrwcitFrhu4OyX3T98_zCQ4BYM9FARfA/viewform
The Student Behavior Sheet can be filled out for the student by multiple different people or the one tho knows them best.
This can also be filled out by the school team at the same time.
Check only the behaviors that are consistent (behaviors that we see daily, not occasionally).
Principal will be contacted by Dr. DeLaine with next steps.
There is a process for students who are 504 and IEP if they are recommended to attend.
If a student doesn't meet the criteria for School of Scholars, school teams need to reconvene to look at supports for the student that was referred.
Reach out to Fred Latschar, Angela Guy, or Angela Rogers for more help if you need support with this process.
