Facilitator Responsibilities:
Post State Health Benefit Program (SHBP) materials in the break room or teacher work room.
Forward emails to school staff that are provided by SHBP
Distribute flyers provided by the SHBP.
Encourage school staff to complete SHBP BeWell Program to earn 480 Incentive Credits each year.
State Health Benefit Plan Rewards Program
• Registration through BeWellSHBP.com and completion of the RealAge Test are required before points can be earned.
• Points will not be awarded for well-being activities until completion of the RealAge Test.
• ALL points earned in 2025 must be redeemed through Sharecare’s Redemption Center (points will not be sent automatically to your health plan). To redeem your earned 2025 points, close this window and select the rewards tab in your reward center.
• Points are saved in the Redemption Center until you choose to redeem them.
• Points may be redeemed in increments of 120 (up to a maximum of 480) well-being incentive credits. Members and covered spouses may each earn up to 480 points, for a household maximum of 960 points. Credits will be available within 30 days of redemption and will be deposited into your HRA/MIA/HIA account.
• You will not be able to select the Sharecare Rewards Visa® Prepaid Card if you begin redeeming points toward your HRA/MIA/HIA credits.
•You must complete all well-being activities totaling 480 points in order to select the one time $150 Visa Prepaid Card. The Visa Prepaid Card can be used anywhere Visa is accepted. The Visa Prepaid Card will be physically mailed within 8 weeks of redemption. The Visa Prepaid Cards expire 12 months after the card issuance date.
• Any points that have not been redeemed by midnight on 12/15/2025 will roll over to your HRA/MIA/HIA account.
