The Henry County Mental Health and Wellness Department operates under funding from Title IVA. As the Mental Health and Wellness Facilitator, you are the Title IVA Inventory Monitor for your school. This means that you will be responsible for updating Inventory Logs for the school, labeling Title IVa funded equipment prior to distribution with district-provided barcodes, participating in two physical inventories per year, ensuring strong internal inventory processes, and completing disposition forms for lost, stolen, or obsolete equipment.
Title IVa equipment falls into the category of what is given to the Mental Health and Wellness Facilitator to be used at the school. Examples of this would be the Big Joe bean bag chairs, weighted teddy bears, and books. These items would remain at the school even if the facilitator were to leave. Some items, like fidgets, journals, or art supplies are considered consumable, and, while we must be mindful of keeping up with them, they do not have to be tracked on the inventory form.

The following sheet details the items that should be located at your school as part of the FY 2022/2023 Wellness Toolkit Grant. More information is available in your specific school inventory folder that you will receive the link from Darlene Minshew.
This is a sample Check In/Check Out Form that you can use to keep track of items that you may loan out to teachers. Our toolkits contain items such as wiggle seats that teachers may use in their classrooms. Be sure that you can account for these items when it is time for inventory.
FAQ
I'm new to my school, and I can't find all the items on my sheet. What do I do? First, double-check with Darlene Minshew to see if there were any issues with ordering, inventory, or disposition of any of those items. Second, ask your admin if they are aware of where these items may be. They may have an idea if something was borrowed or stored. If you still can't find them, contact Darlene to let her know.
I loaned out a fidget to a student that needed something, and they didn't bring it back. Fidgets, journals, and books are all considered consumable items. While we try to be diligent with those items, it's understandable that they may break or disappear. It's ok. You don't need to mark that on your inventory sheet.
