The ADOPIE Nursing Process consists of an assessment, diagnosis, outcomes, planning, implementation and evaluation.
The A in ADOPIE stands for assessment! This step includes:
Collecting subjective data (what the patient says)
Collecting objective data (what is observed)
Determine past medical history
Perform assessment (physical assessment and vital signs)
Obtain the whole picture of the patient's condition and health status
The D in ADOPIE stands for Diagnosis. This step includes:
Analyzing assessment data
Determining nursing problem
Formulating a nursing diagnosis
The O of ADOPIE stands for outcome. This step includes determining goals that are:
Measurable
Realistic
Specific
Time-limited
Patient centered
Should be achieved following nursing interventions
The P in ADOPIE stands for planning. This step includes:
Developing a care plan
Prioritizing patient problems
Choosing evidence-based actions
Involving the patient in the plan
Interprofessional collaboration
The I in ADOPIE stands for implementation. This step includes:
Following the care plan, carrying out planned interventions
Educating the patient
Interprofessional collaboration, coordinating care
Providing treatments
The E in ADOPIE stands for evaluation. This step includes:
Determining if the goals were met
If goals were met, encourage continuing the plan or stopping if patient no longer needs to continue
If goals were NOT met, modify the plan and repeat.
