UNDER DEVELOPMENT - Online Request for Assistance Portal

This is the way in which Requests for Assistance (RfAs) to the Single Point of Access are made. Please see the button at the top of the SPA Home Page to start are new Request. 

Requesters will use an online form to request assistance from the SPA Services. The form will ask all the pertinent questions as relates to the services requested and will also allow requesters to upload supporting documents. 

Please be aware that each page has a 20 minute timeout. If 20 minutes elaspes and 'Back' or 'Next' are not clicked, the form will expire. 

To avoid frustration and delay please ensure that you have all the information you need, including appropriate consent and enough time to fully complete the form. If this is not possible, you might consider using a saveable word document to enter information into until you have time to complete the online form. To make this easier we have supplied a word document template for the lengthier sections of the Request form.
