Work Permit

Here is the paperwork for a work permit, necessary for 14 and 15 year old's who want to hold a job. Once you get your job, have your employer fill out their section. Your School counselor can provided the documentation required for the Work Permit Application, and a letter of consent (referred to as the "Principal's Statement").

You will then take your paperwork to the ROE to finalize your permit. 

***The ROE is now located in the Health & Human Services Building at 2116 N. Sheridan Rd.***

You must go with a parent after 2:30pm on a weekday.
