What is a Citizen's Charter?

A Citizen's Charter is an official document that tells the public what services a government office provides, how to access those services, how long they take, what requirements are needed, any fees to be paid, and who is responsible for delivering the service. It serves as a commitment by the government agency to provide efficient, transparent, and accountable public service.

Frontline Services

1. Enrollment of Junior High School Learners (Grades 7–10)

Assists eligible learners in enrolling for the current school year through a learner-friendly and efficient enrollment process.

Procedures

Submit the accomplished Basic Education Enrollment Form (BEEF) together with all required documentary requirements to the designated enrollment personnel.
School personnel review and verify the submitted documents to ensure completeness and authenticity. If any requirements are lacking, the learner or parent/guardian will be informed accordingly.
Learner information is encoded or updated in the Learner Information System (LIS) and other relevant school records.
The School Head or designated Enrollment Focal Person reviews and approves the enrollment upon successful verification of all requirements.
The learner or parent/guardian is informed of the successful enrollment, including the assigned section/class and other important enrollment details.

2. Enrollment of Senior High School Learners (Grades 11–12)

Facilitates the enrollment of incoming and continuing Senior High School learners, including track and strand confirmation.

Procedures

Submit the accomplished Basic Education Enrollment Form (BEEF) together with the required documentary requirements, including the learner's credentials and other supporting documents.
School personnel verify the learner's eligibility for Senior High School enrollment and confirm the preferred track and strand based on available slots and applicable admission requirements.
The learner's information is encoded or updated in the Learner Information System (LIS) and other relevant school records.
The School Head or designated Enrollment Focal Person assigns the learner to the appropriate section based on the approved track/strand and available class capacity.
The learner or parent/guardian receives confirmation of enrollment, including the assigned track, strand, section, and other important enrollment information.

3. Admission of Transferees

Assists learners transferring from another school to continue their education without unnecessary delay.

Procedures

Submit the required transfer credentials and other documentary requirements to the designated school personnel.
School personnel review and verify the authenticity and completeness of the submitted documents to determine the learner's eligibility for transfer.
The school evaluates the learner's appropriate grade level and confirms the availability of slots in the requested grade level or program.
Upon approval, the learner's admission and enrollment are processed, and the learner's information is encoded or updated in the Learner Information System (LIS) and other relevant school records.
The learner or parent/guardian is informed of the successful admission and provided with the assigned section/class and other pertinent enrollment information.

4. Admission of Balik-Aral (Returning Learners)

Provides returning learners with the opportunity to resume their basic education.

Procedures

1. Submit enrollment form and available school records.

2. School evaluates previous educational records.

3. Determine appropriate grade level.

4. Process enrollment.

5. Issue class assignment.

5. Issuance of Certificate of Enrollment

Issues an official certification confirming that a learner is currently enrolled in the school.

Procedures

1. Submit request form or letter.

2. Verify learner information.

3. Prepare the certificate.

4. School Head signs the certificate.

5. Release the certificate to the requester.

6. Issuance of Good Moral Certificate

Provides certification of a learner's good moral character for scholarship, employment, transfer, or other legitimate purposes.

Procedures

1. Submit request.

2. Verify learner's disciplinary records.

3. Adviser or designated personnel prepares the certificate.

4. School Head signs.

5. Release the certificate.

7. Issuance of School Records (SF9, SF10, Diploma, Certificate of Completion/Graduation)

Provides certified copies of school records for educational, employment, or legal purposes.

Procedures

1. Submit request and valid identification.

2. Verify records.

3. Prepare requested document.

4. School Head signs or certifies the document.

5. Release to the requester and record the transaction.

8. Issuance of School ID

This service provides official identification cards (School ID) to enrolled learners for school identification, security, and access to school services and facilities.

Procedures

For New / Incoming Students (Initial Issuance):

Learner is officially enrolled in the school.
School personnel verifies enrollment in the Learner Information System (LIS).
Student submits ID picture (if required by the school).
School prepares and prints the School ID.
School Head or authorized personnel validates/approves the ID.
School ID is released to the learner.

For Replacement of Lost/Damaged ID:

Student secures and submits a request form or letter.
Present affidavit of loss (if required by school policy).
School verifies student identity and enrollment status.
Processing fee (if applicable under school policy) is collected and officially receipted.
School prints replacement ID.
ID is signed/validated and released.

Typical Requirements:

Officially enrolled learner status (verified via LIS)
Recent 1x1 or 2x2 ID photo (white background, if required)
Request form / letter (for replacement cases)
Affidavit of loss (for lost ID)

Processing Time (Suggested Standard)

Initial issuance: 3-5 working days after enrollment validation
Replacement ID: 3-5 working days upon completion of requirements

Fees

School ID is issued free of charge for first-time release. However, replacement or renewal of a lost or damaged ID may be subject to a corresponding fee, in accordance with applicable school policies and allowable MOOE expenses.

9. Authentication/Certification of School Documents

Certifies that copies of school-issued documents are true copies of the originals kept by the school.

Procedures

1. Present original document and photocopy.

2. Verify authenticity.

3. Affix certification stamp and signature.

4. Record the transaction.

5. Release certified documents.

10. Receiving and Releasing Official Communications and Documents

Receives, records, routes, and releases official communications addressed to or issued by the school.

Procedures

1. Submit document at the receiving desk.

2. Log the document.

3. Route to the concerned office.

4. Process action required.

5. Release reply or processed document, if applicable.

11. Receiving Complaints, Suggestions, and Feedback

Provides stakeholders with an accessible mechanism for submitting complaints, suggestions, commendations, and other feedback to improve school services.

Procedures

1. Submit feedback through the designated channel.

2. Record the concern.

3. Forward to the concerned office.

4. Evaluate and act on the concern.

5. Inform the client of the action taken, when appropriate.

12. Responding to General Inquiries

Assists clients seeking information about school programs, policies, enrollment, and other school-related matters.

Procedures

1. Approach the Information/Receiving Desk.

2. State the inquiry.

3. Personnel provide information or refer the client to the appropriate office.

4. If necessary, record the inquiry for follow-up.

5. Close the transaction after the concern is addressed.
