Guidelines for online enrollment

1. Online enrollment: access the link or scan the QR code (for regular/old Students only)

Note: You must have a google account preferably using your real name (not alias or pseudo-name) and you need to login to your email account before you open the online enrollment. If you don't have a google account yet, you may create one. Parents’ google account can be used, due to age restrictions.

2. On- Site Enrollment:

for Transferees, Balik-aral, PEPT passer, ALS completer

• Get an enrollment form at the designated drop box at the school,

• Fill out the form completely,

• Attach the documentary requirements,

• Place the documents inside a long brown envelop,

• Drop at the designated drop box

3. At the barangay LGU office (for barangays more than 3 km away from CHS)

• The Parents/APOR Learners shall secure an enrollment form at the office of the Barangay

• Fill out the form completely,

• Attach the documentary requirements,

• Place the documents inside a long brown envelop,

• Drop at the designated drop box at the school

Documentary Requirements

Old/Regular Students/Balik-Aral

· Filled enrollment form

· SF9 (Form 138 - Report Card)

· Long brown envelope

ALS Completer

· Filled enrollment form

· Certification from DO ALS Coordinator

· Result of A&E Test

· Birth Certificate

· 2 pcs 1X1 picture

· Long brown envelope

Transferees, Incoming Grade 7, PEPT Passer

· Filled enrollment form

· Original SF 9 Grade 6 (Form 138 - Report Card)

· Certificate of Good Moral Character

· Birth Certificate

· Affidavit of undertaking

· Result of PEPT (if applicable)

· PSA Birth Certificate

· 2 pcs 1X1 picture

· Long brown envelope

Incoming new student for Open High School

· Filled enrollment form

· ILRT result (see Mam Marianne De Castro)

· Informal Reading Inventory (IRI) test,

· Interview of Parent for Learner Profile

· Original SF 9 Grade 6 (Form 138 - Report Card)

· Certificate of Good Moral Character

· Affidavit of undertaking

· Result of PEPT or A&E Test (if applicable)

· PSA Birth Certificate

· 2 pcs 1X1 picture

· Long brown envelope
