Guidelines for online enrollment

Online enrollment: access the link or scan the QR code (for regular/old Students only)

Note: You must have a google account preferably using your real name (not alias or pseudo-name) and you need to login to your email account before you open the online enrollment. If you don't have a google account yet, you may create one. Parents’ google account can be used, due to age restrictions.

On- Site Enrollment:

for Transferees, Balik-aral, PEPT passer, ALS completer

Get an enrollment form at the designated drop box at the school,
Fill out the form completely,
Attach the documentary requirements,
Place the documents inside a long brown envelop,
Drop at the designated drop box

At the barangay LGU office (for barangays more than 3 km away from CHS)

The Parents/APOR Learners shall secure an enrollment form at the office of the Barangay

Admission Requirements for Special Program in Sports

------this Program is not implemented this time of Pandemic-----------

· Open to all high school-aged students enrolled in Philippine Schools

· The student of SPS must:

* Have the potential/skills in more than one of the school sports offered;

* Have participated in sports competition as attested by the elementary school principal;

* Pass the physical fitness test, and

* Submit the following documents upon enrollment:

- Report Card (F138)

- Medical certificate administered by Government Physician

- Parental consent stating that the student may undergo rigid training in specialized sports; and

- Certificate of Good Moral certified

by the Elementary School Principal

Open High School Qualification for Admission and Procedure

Open to all Filipino learners who can demonstrate capacity for independent learning, and are willing to undertake self-directed learning.

a. Entrance Requirement:

> High School Report Card (in case of drop-out)

> Elementary Report Card (for high school entrants) PEPT qualifying certificate

b. Must Pass the Assessments

> Independent Learning Readiness Test (ILRT) > Informal Reading Inventory (IRI)

c. Interview with the Parent / Guardian

d. Sign a Contract Agreement (Student and Parent/Guardian)

e. Must attend the Orientation Program together with the Parent/Guardian

*Note: Documentary requirements at regular classes also applies to OHSP

Delivery Mode

· Maximum of 20 students per classroom

· Meeting shall be every Saturday from June to March

· Shall follow the existing K to 12 Curriculum

· Learning shall be flexible, multi-channelled, and learning directed

· Retention in the program is for a maximum period of six (6) years

· Option to be mainstreamed in the regular program any time within the period of study
