Below is one of Adriel's care plans for a patient of his who suffered from pneumonia.
Adriel believes in creating care plans to identify the existing needs of his patient, as well as recognize potential needs and risks. They require the integration of academic knowledge with clinical experience in order to direct nursing care. For example, research would be done using nursing textbooks to create the pathophysiology portion and the concept map section. The care plan below also goes into detail about collaborating with other health care professionals from a variety of disciplines such as respiratory therapists, physical therapists and physicians in order to provide more comprehensive and holistic care. In this care plan, physicians write the prescription for a respiratory medication, the RT administers it, and the nurse monitors for adverse effects. When used correctly, care plans can be a valuable tool that elevates the nurse's ability to provide care.
