Health Information

Each student that had any of the conditions listed on the health screen checked automatically has a tag, HEALTH ISSUE.


Additionally, there are tags for the following related to Health Conditions:

  • Allergy NS: Health Screen - Non-Severe allergy was answered yes.
  • Allergy S: Health Screen - Severe allergy was answered yes.
  • Asthma: Health Screen- Asthma was answered yes.
  • Diabetes: Health Screen; If answered yes to diabetes
  • Immunization Records: Record can be tagged when Immunization records are still needed on student.
  • Medicine: Health Screen: Any question that is answered requires medication given at school.
  • Seizure: Health Screen: If answered yes to seizure.
  • Vision Issue: Health Screen- both forms: If answer yes to Vision Problems.


There are three workspaces, the New Student Registration, Returning Student Registration, Early Childhood/Speech Registration so you will need to search all three workspaces for students with any of these tags.

Finding a List of All Students with Health Issue Tag:

To see a list of all students with the HEALTH ISSUE tag (or any other medical tag you'd like), enter the Submission workspace (new or returning) and click Health Issue under the Tag drop down menu.

The list will reload and show only students with the Health Issue tag.


If you'd like to download this list, click the Tasks drop down menu and select Export.

A new window will appear and you can select which field you would like exported. The medical fields begin about 2/3 of the way down the list and all begin with med_. Check the box in front of any that you would like included. The choose either Selected Only or All Records Found (depending on which you need).

When the export is complete you will see a link in the box displayed. Click on the Download Data link to download and open the Excel file.

To download Emergency Forms or Health Information for each student {aka: Blue Cards}:

Enter the Submission workspace (new or returning) and be sure All Submissions are selected under View.

Under the Tasks menu, select Print Forms.

Select the Health Form from the list of choices you would like to print (all Medical forms are checked in the image below) and either All Found or Selected Only.

When the export is complete you will see a link in the box displayed. Click the View Forms link.

You can then save or print the forms using the icons in the top right corner.

To save the forms, click View Forms and after they open in the browser window, click the Save icon in the bottom right hand corner.
