Any medication needed during the school day requires a permission form to be completed. Forms can be obtained from the school nurse. 

Over-The-Counter Medication:

The form must be completed and signed by the parent and submitted to the school nurse.

Prescription Medication:

The form must be completed by a physician and signed by both the physician and parent, then turned into the school nurse.

**All prescription medication must be kept in the nurse's office except for inhalers, EpiPens, and insulin.
