Staff and volunteers are responsible for reporting any Citipointe related injuries, illnesses or incidents where a person was injured, could have been injured, or equipment was damaged. 

Citipointe Church has an active reporting, recording, investigation and corrective action process.  The terms of incidents and injuries in this context includes all “near miss” events, illnesses and injury, and events related to Citipointe that harmed or might have harmed someone at or during a Citipointe event. 

How do I know whether to submit an Accident, Incident and Injury Report or not?

The Accident, Incident and Injury report should be used for incidents where

• first aid was administered 

• Emergency services were called

• a person was fine at the time but then later shows with an injury 

If a person has injured themselves and first aid was applied, or the injury was significant enough that you are concerned, you should fill in the Accident, Incident and Injury Report.
