What is the Care Team?

The Care Team is a multidisciplinary team that meets regularly to evaluate behaviors by college students that may be hindering their success at Champlain, behaviors that may be perceived as threatening, harming or disruptive to the student, to others or to both, and coordinate an appropriate response. The Care Team does not respond directly to emergencies and, in an emergency Campus Public Safety and/or 911 should be contacted.

Who Should be Referred?

Faculty, staff, friends and family are frequently in a position to observe student behavior that may indicate a challenge to a student's success, a student in distress, or potentially compromised personal well-being. If you have a concern about a student who is displaying behaviors that may interfere with the student's ability to be successful in the college environment, please make a referral to the Care Team. A Care Team referral can also be made if you are concerned that a student's behavior or well-being are negatively impacting those around the student.

How to submit a report?

Simply click on the link to the Student of Concern form and complete the items requested.  Provide as much information as you can (including any interventions you have attempted).  You can copy portions of an email and paste into the document.  Please provide as much information you have available to assist the Team.

What Happens After I submit a Referral to the Care Team?

Referrals to the Care Team are reviewed and acted upon as soon as possible after submission, generally within one business day of submission. You may be contacted by a member of the Care Team to provide any additional information regarding your referral. After submitting a referral, you can expect to receive a notice that your referral has been submitted; this is your confirmation that the Care Team will be receiving your referral. Due to student privacy and the volume of referrals, the Care Team is not able to provide an individualized response to each reporter after receiving a referral.
