Appendix A - Self-Assessment Protocol

Before leaving home, staff, students, parents/guardians and essential visitors who will access the school for work or education must self screen for symptoms each day prior to entering the school using the attached tool (see Appendix B - Self Assessment Checklist).

Staff will receive notification to complete the self assessment daily when logging into their board account and via their board email account.

Essential Visitors (including parents/guardians) to any school site or the CEC will be asked to complete the self assessment using the KIOSK at the front entrance prior to entry.

Students are required to complete the self assessment at home on a daily basis. The self-assessment tool will be distributed through School Messenger with the expectation that the self-assessment be conducted on a daily basis prior to boarding a school bus or entering a school.

Schools must keep records of children’s known pre-existing conditions. If a child develops new or worsening symptoms at home or during the school day that could be caused by either COVID-19 or by a known pre-existing condition (e.g., allergies), the child should be tested for COVID-19 to confirm that it is not the source of their symptoms before entering or returning to school. Staff and other adults must assess themselves daily, using the Ontario Public Health COVID-19 Self-Assessment, for symptoms of common cold, influenza, or COVID-19 prior to entering the school. If staff or any adult has any symptoms, they must not enter the school. School administrators must ensure school staff and other adults entering the school are aware of their responsibility to assess themselves daily for symptoms of common cold, influenza, COVID-19 or other infectious respiratory disease prior to entering the school.

Anyone that reports symptoms should be directed to stay home, and contact their health care provider to seek advice and guidance as to whether COVID testing is required.
