This template can be utilized while preparing for your LEA monitoring interview. This is not a requirement but has proven to be a helpful tool for other liaisons.

Region 2 Monitoring Tips:
Review the monitoring prep guide. The PA ECYEH Monitoring Tool and Virtual LEA Monitoring Visit sections provide in-depth descriptions of the required documentation and evidence needed during the interview process.
Once you receive notification that your LEA is up for monitoring, begin compiling required and supporting documentation as soon as possible. Keep all documentation together in a folder on your computer to make uploading files to the monitoring system easier. All files need to be uploaded at least 4 days prior to the monitoring visit.
It may be helpful to utilize an online application (e.g., OneDrive, Google Drive, or Dropbox) or simply a file folder on your desktop to organize your documentation before uploading it to the data site. Organize materials into separate digital folders by the corresponding question number on the tool. We recommend collecting all of your documentation to upload everything at once.
Do not show confidential information to the monitor (i.e., redact the child’s name, birthdate, address, etc.).
Check the dates on all documentation. You must show the most up-to-date information. The monitor will request documentation from this year. You may submit documentation from previous years if necessary.
Be sure to communicate with your monitor to finalize the date and time of your monitoring meeting.
