Residency Questionnaires are one of the most helpful strategies for screening all potential students applying to enroll in a Local Education Agency (LEA). These questionnaires are encouraged to be included in typical enrollment paperwork. If flagged as potentially McKinney-Vento eligible, those students or families should be referred to the LEA homeless liaison for further eligibility determinations.
LEAs may use these samples, or adapt them as needed.
Homeless Liaisons are required to reevaluate McKinney-Vento eligibility annually. Many families and students may be eligible for multiple years. Liaisons are encouraged to utilize mid-year and/or end-of-year check-in forms to help with this process. Local Education Agencies may use these samples as-is, or adapt them as needed.
In the event that a Local Education Agency has deemed a family ineligible for McKinney-Vento services, contrary to the interpretation of the family, the family must be provided this notice in writing and provided information about their right to appeal this decision. See the Dispute Resolution section for more information.
