The 2 PC Track residents each year act as continuity clinic practice partners throughout residency.
Early exposure to the breadth of available primary care opportunities through:
Primary care immersion at the beginning of intern year.
Rotations through primary care selectives.
Early focus on quality improvement, scholarly activity, and leadership opportunities.
Subspecialty experiences focused on outpatient evaluation to optimize primary care management prior to subspecialty referral.
Focused education and skill development on transitions of care, patient panel continuity, and the business and leadership aspects of independent practice.

Monthly Primary Care IM Case Report & Practice Population Care Committees.
In-depth exposure to transitions of care by pairing hospital medicine rotations with transition clinics and visits to skilled nursing facilities, assisted living facilities and home visits to follow up on the care of your discharged patients.
Subspecialty Ambulatory Long Block— weekly continuity with various subspecialists. This longitudinal block is focused on learning the outpatient approach to common subspecialty issues and the appropriate work up prior to referral.
Additional electives to help identify an area of focus, based on your career goals, for 2nd continuity clinic site.
Ongoing exposure to inpatient medicine as team leader and educator, but less focus on ICU -- just one month per year in years 2 & 3. Instead, more focus on primary care leadership and advocacy.

Where is the primary care continuity site?
Answer:
The primary continuity clinic site is at the BUMCP Internal Medicine Center next to the main teaching hospital. This practice has a diverse patient population both in ethnicity and socioeconomic circumstances.
As we expand the Primary Care Track to 4 resident position per year, the brand new VA Ambulatory Clinic (built in 2022) will become the primary continuity clinic site for half of the PC Track residents.
You will begin a 2nd continuity clinic site -- based on your area of interest -- midway through your second year.
